In accordance with Local Rule 47.02, the Court may submit written interrogatories to the jury venire for each term of court. Only attorneys and parties with cases on the relevant trial roster are allowed access to this information. Any other person submitting a request for this information must petition the court for approval. A Juror Questionnaire/List Request Form must be submitted to the Clerk’s Office for approval to review or purchase the questionnaires and the jury list. Forms are also available at the Clerk’s Office. The approved form will be returned to the requesting party to present to the printer under contract for the appropriate division listed below. The request can be processed via fax with the Clerk’s Office and the printing company. The original approved request form shall be maintained in the Court's jury venire folder. Copies of the responses and the jury list are available for purchase beginning seven working days prior to jury selection. The originals of the responses are available for review at the Clerk’s Office during the same period. These interrogatories will not be repeated at jury selection.
As indicated on the request form, the information contained in the juror questionnaires and jury list is confidential and is to be used solely for the purpose of evaluating potential jurors for a case and is not to be used or disseminated for any other purpose. The questionnaires and list must remain in the custody of the ordering party. All copies of the questionnaires and jury list, on paper or disk, should be destroyed at the conclusion of the case and shall not reside on any computer.
